Service Description
Our person-centred functional capacity assessments are completed by occupational therapists to help determine the right level of support and funding needed to enable you to achieve your goals. This comprehensive assessment involves the occupational therapist finding out how you: - complete your personal care and domestic tasks - mobilise - communicate and interaction - learn - make decisions - manage your well-being This assessment and report will require approximately *16 hours of funding. These hours include both face to face and non-face-to-face time to complete: - an interview with you - standardised assessments (including scoring and interpretation) - liaison with others in your care team - report writing - clinical note documentation - travel time to/from Moorebank * Please note you will only be charged for actual time taken.
Cancellation Policy
You are required to pay for any services provided by Enable Me OT that you have consented to. If payment is not received within 2 weeks of being invoiced, debt collection may commence. If you no longer require the services of Enable Me OT you will be required to advise your treating therapist atleast 48 hours prior to your appointment. If no notice is given, you will be charged for your appointment, minus travel time. Please note travel time will be charged if the therapist has travelled to your appointment location.
