Service Description

Our occupational therapy Support at Home services are delivered in line with the Australian Government’s Support at Home program, which replaces Home Care Packages and is designed to help older people live safely, independently, and with dignity in their own homes for as long as possible. We provide practical, person-centred OT support that focuses on what matters most to you — your routines, your environment, and your goals. Occupational therapy through Support at Home focuses on enabling everyday activities and maintaining independence within the home. This may include support with: - Daily living activities such as showering, dressing, meal preparation, and mobility - Fatigue management and energy conservation - Falls prevention and home safety - Sensory and environmental set-up to support comfort and regulation - Assistive technology and equipment recommendations - Establishing or adapting routines that support confidence and wellbeing - Supporting carers with practical strategies and education All support is tailored to your needs, preferences, and pace. We work collaboratively, taking the time to understand how you use your space and what feels important in your daily life. Rather than rushing change, we prioritise safety, comfort, and sustainable support. We also recognise that support at home often involves a wider team. We work alongside families, carers, care coordinators, and other providers to ensure supports are aligned and meaningful. Our services may also include: - Functional assessments and written recommendations - Assistive technology assessments and trials - Home modification advice - Reports to support ongoing care planning
