Service Description
A Supported Independent Living Assessment or SIL Assessment as it is commonly known provides information to the NDIS on the type of supports that you need to help you live at home. These supports may include: help with personal care, meal preparation, social skills, or medication or health care management, as well as help to action any behaviour support plans or supervision you may need, or to get to and from community access activities. This assessment and report may take up to *20 hours of funding. These hours include both face to face and non-face-to-face time to complete: - interview - standardised assessments (including scoring and interpretation) - liaison with others in your care team - report writing - clinical note documentation - travel time to/from Moorebank * Please note you will only be charged for actual time taken.
Cancellation Policy
Please note: You are required to pay for any services that you consent to undertake with Enable Me Occupational Therapy. If you no longer require services, you or your carer/advocate must advise Enable Me Occupational Therapy atleast 48 hours before your appointment time. If notice is not provided you will be charged for your appointment, minus travel costs, unless the occupational therapist has travelled to your location.
